FAQs

How often you should have a PSA test depends on your age and your personal level of prostate cancer risk. Most men with average risk are advised to have a PSA test once a year from age 50. Men with higher risk, such as Black men or those with a family history, should have a PSA test every year from age 45. Men with very high risk may benefit from starting yearly PSA tests from age 40, especially if prostate cancer runs in your family. 

What your PSA level should be depends on your age and prostate health. Typical PSA guidance includes:

  • PSA under 1.0 ng/mL: very low risk
  • PSA 1–4 ng/mL: can be normal depending on age
  • PSA above 4.0 ng/mL: considered abnormal and usually requires further investigation

It’s important to remember:

  • A normal PSA does not completely rule out cancer
  • A high PSA does not automatically mean cancer
  • PSA naturally increases with age
  • The trend over time is often more meaningful than a single result

It’s also important to consider PSA velocity, meaning how quickly your PSA level changes over time. A sudden rise can be more concerning than one mildly elevated result. 

To prepare for a PSA test properly and ensure the most accurate result, you should:

  • void ejaculation for 48 hours before your test
  • void vigorous exercise or cycling beforehand
  • Postpone the test if you currently have a urinary infection. 

You should also let us know if you have recently had any prostate procedures. There is no need to fast, so you can eat and drink normally before your PSA blood test.

Total PSA, Free PSA and the PSA ratio each give different information about your prostate health. Total PSA measures the overall amount of prostate-specific antigen in your blood. Free PSA measures the PSA that is not attached to proteins. The PSA ratio (free-to-total percentage) compares these two values. A lower PSA ratio can sometimes suggest a higher chance of prostate cancer, while a higher ratio often indicates a benign condition. A full prostate profile uses all three values to create a clearer picture.

Your PSA level might be elevated even if you do not have prostate cancer because many benign conditions can raise PSA. These include ageing, having a larger prostate, prostate inflammation (prostatitis), urinary infections, recent ejaculation, vigorous exercise, cycling or recent prostate procedures. A raised PSA simply indicates that something may need further investigation.
